Position Overview:

As a Community Support Worker, you can make a real difference in the lives of people who are facing challenges of daily living within a community care setting. In this role, every day grants you a unique experience and adventure, empowering people to live their best lives, and achieve their goals.

Responsibilities:

  • Conduct thorough assessments and intakes for families and single women entering the program.
  • Continuously evaluate clients' needs and progress, adapting case plans as necessary.
  • Develop individualised case plans that address clients' specific needs.
  • Ensure that case plans are dynamic and updated regularly to reflect changes in clients' situations.
  • Oversee properties, conducting regular property inspections and completing entry and exit reports.
  • Set clear plans for property maintenance and client responsibilities.
  • Advocate heavily for clients to negotiate housing stability, mediating between clients and real estate personnel.
  • Serve as the primary intervention resource for clients during crises.
  • Facilitate access to critical services such as mental health support, financial aid, counselling, and domestic violence resources.
  • Coordinate with Centrelink to ensure clients receive necessary finanical assistance.
  • Manage referrals from various sources, ensuring clients receive appropriate support.
  • Collaborate with other government bodies, including Police, Child Services, Public Trust, Guardian Services, Corrections, Youth Justice, and education systems.
  • Develop and implement chore charts and other skill-building activities to enhance clients' independence.
  • Provide budgeting assistance, helping clients to manage their finances effectively.
  • Deomonstrate judgement in identifying when to manage issues internally and when to refer clients to external services.
What You Need:

✅ Hold an Australian Certificate IV or higher in Mental Health, Social Work or Community Services
✅ Australian Drivers License (Full Licence)
✅ NDIS Worker Check
✅ Blue Card
✅ Current First Aid & CPR certificate
✅ Own or have Access to a Registered Vehicle
✅ Comprehensive Car Insurance
✅ The ability to work a mixture of day, night and weekend shifts as required
